    • Submit an FML case in Service Hub to determine eligibility

  • Supplemental, Personal, and Personal Medical Leave of Absences

  • Worker’s Compensation Injuries (Sedgwick)

    • Worker’s Comp Handbook

    • Workers Compensation guidance provided by Be Well at Work

    • The Employee and Supervisor and/or witness to the injury should complete the IIR for the incident as soon as possible

    • Create a Service Hub case requesting FML if the employee is unable to work after the injury and cannot be reasonably accommodated

    • If the employee is able to work with accommodations please submit a Reasonable Accommodations Case in Service Hub and note the injury is work related
